RN, Case Manager, Home Health

*POSITION SUMMARY: *The incumbent is responsible for the nursing care provided to patients in the home; the assessment of patients; and the plan for nursing care.
*ESSENTIAL FUNCTION:* 1.
Makes the initial home visit and completes the admission paperwork, assessment and medication reconciliation when the patient is admitted over the weekend, holiday or after hours.
2.
Coordinates the services of and supervises other health professionals and home care aides.
3.
Provides appropriate skilled nursing services, following policies and procedures.
4.
Educates the patient and caregiver as per identified educational needs, and records the response to the education.
5.
Informs the physician and other personnel of changes in the patient’s condition.
6.
Performs call for the department as per schedule, and handles emergencies, and after hour admissions & patient needs without difficulty.
7.
Completes the required paper work, reports, and medical record documents in a timely and efficient manner.
8.
Rotates weekend and holiday call duty as scheduled.
9.
Performs other duties as assigned by the Home Health Director.
*REQUIREMENTS:* A.
Education and Training: Graduate from an accredited school of nursing resulting in the eligibility to sit for the Registered Nurse exam.
B.
Experience: Must have at least three years of Home Health nursing experience.
Prefer at least one year of supervisory experience C.
Licensure, Registration, Certification: Licensed as a Registered Nurse in the State of Arkansas.
CPR Certified.
