- Report all observed accidents/incidents promptly.
- Use only authorized abbreviations when recording information.
- Report changes in residents' conditions to the Charge/Staff Nurse promptly.
**Admission, Transfer, and Discharge Functions:** - Ensure residents' rooms are ready for receiving them.
- Greet and escort residents to their rooms, introducing them to roommates and others as appropriate.
- Make residents comfortable and inventory their personal possessions.
- Assist residents with packing and unpacking when transferring rooms or being discharged.
- Transport residents to new rooms or the receiving area.
**Personnel Functions:** - Follow work assignments and schedules.
- Perform all assigned tasks according to established policies and procedures.
- Notify the facility of late arrival or absence.
- Report residents' complaints and grievances.
- Cooperate with inter-department personnel for efficient nursing services.
- Create and maintain a positive and calm environment throughout the unit and shift.
- Attend scheduled meetings to identify and correct problem areas.
**Personal Nursing Care Functions:** - Participate in and receive the nursing report upon duty.
- Assist residents with daily dental and mouth care, give or assist with bath functions, backrubs, dressing/undressing, hair care, nail care, shaving, and keeping residents dry.
- Make beds, position bedfast residents, change bed linens, and assist with bowel and bladder functions.
- Collect specimens, assist with medical tests, and prepare residents for physical examinations.
- Assist with lifting, turning, moving, positioning, and transporting residents.
- Provide restorative and rehabilitative procedures, eye and ear care, and measure/record vital signs.
**Special Nursing Care Functions:** - Observe and report the presence of pressure areas and skin breakdowns.
- Provide daily perineal care and turn bedfast residents at least every two (2) hours.
- Perform diabetic urine testing, observe disoriented and comatose residents, and report any changes in room conditions.
- Turn over all medications found in residents' rooms to the Charge/Staff Nurse.
**Food Service Functions:** - Prepare residents for meals and serve food trays, assist with feeding, and record residents' food/fluid intake.
- Keep residents' water pitchers clean and filled, serve snacks, and perform after-meal care.
- Check rooms for food articles, ensuring adherence to dietary allowances.
**Staff Development:** - Attend and participate in scheduled training, educational classes, and orientation programs.
**Safety and Sanitation:** - Restrain residents as instructed, release restraints periodically for exercises and other activities.
- Maintain records of restrained residents.
- Report missing residents promptly and follow established safety precautions.
- Keep floors dry, store supplies and equipment properly, and follow smoking regulations.
- Keep bedside rails up when residents are in bed and wash hands before and after resident services.
- Follow isolation precautions, use gowns and gloves as indicated, and adhere to Universal Precautions.
**Equipment and Supply Functions:** - Use only trained equipment and operate it safely.
- Use only necessary equipment and supplies to avoid wastefulness.
- Report defective equipment and inform the Charge/Staff Nurse of needs.
**Care Plan Functions:** - Review care plans daily to ensure appropriate nursing care.
**Residents' Rights Functions:** - Maintain confidentiality of resident care information.
- Treat all residents fairly, with kindness, dignity, and respect.
- Provide nursing care in privacy, knocking before entering residents' rooms.
- Report residents' grievances and complaints to the Charge/Staff Nurse.
**Working Conditions:** - Work in well-lighted and ventilated areas throughout the nursing service area.
- Sit, stand, bend, lift, and move intermittently during working hours.
- Subject to frequent interruptions.
- Involved with residents, personnel, visitors, government agencies/personnel under various conditions.
- Subject to hostile and emotionally upset individuals.
- Communicate with nursing and other department personnel.
- Willing to work beyond normal hours, on weekends, holidays, and temporarily in other positions when necessary.
- Attend and participate in continuing educational programs.
- Subject to falls, burns, odors, and exposure to infectious waste.
- Maintain a liaison with other department supervisors.
**Education Requirements:** - Minimum 8th-grade education.
- Must be a Certified Nursing Assistant or enrolled in a Certified Nursing Assistant Training Program.
**Experience:** - None required, on-the-job training provided.

- Make independent decisions as warranted.
- Deal tactfully with various individuals.
- Minimum age of eighteen (18).
